Expat insurance and health insurance in El Salvador

Quiet, empty street in a Salvadoran colonial town, colourful colonial-style façades, cobblestones, no recognisable people or vehicles, no volcano or identifiable landmark

In short

El Salvador's health system relies on the Ministerio de Salud (MINSAL), which runs a free public network whose national reference facility in San Salvador is the Hospital Nacional Rosales, and on the Instituto Salvadoreño del Seguro Social (ISSS), a mandatory regime for any employee in formal employment (employee contribution of 3% of salary capped at 30 USD per month, employer contribution of 7.5% capped at 75 USD, on a maximum contribution base of 1,000 USD), which also runs its own hospital network including the Hospital Médico Quirúrgico de San Salvador. Among its useful contacts, France Diplomatie lists two private facilities that can handle an emergency: the Hospital de Diagnóstico (two sites in San Salvador) and the Centro Médico Escalón (formerly Hospital de la Mujer), where payment in practice is made via international insurance or a Visa card. No visa is required on entry for a French passport on a stay of less than 90 days, but El Salvador belongs to the CA-4 free movement area with Guatemala, Honduras and Nicaragua, where this period is counted cumulatively across all four countries according to France Diplomatie itself; an extension beyond 90 days remains possible for a serious reason for a 25 USD fee. No social security agreement exists between France and El Salvador: the country does not appear in CLEISS's list of country pages. The rentista (passive income of around 1,460 to 2,000 USD per month according to the sources consulted) and pensionado (pension of around 1,200 to 1,500 USD per month, 1,224 USD according to Consortium Legal) residency statuses carry, in the texts consulted, no documented health insurance requirement, unlike the digital nomad visa (foreign income of at least 1,460 USD per month) which explicitly requires comprehensive international health insurance valid in El Salvador for the entire stay.

Key figures

  • 462 French nationals registered with the consular register in El Salvador in 2025 (MEAE).
  • Life expectancy at birth: 72.30 years in 2024 (World Bank).
  • Health spending: 9.29% of GDP in 2023, around 500.69 USD per capita (World Bank).
  • Visa-free stay of up to 90 days for holders of a French passport, cumulative with the other countries of the CA-4 area (France Diplomatie).

Local healthcare system

El Salvador's health system is organised by the Ministerio de Salud (MINSAL), which runs a public network that is in principle free to access. The Hospital Nacional Rosales, in central San Salvador, is its national reference facility for medicine and surgery for patients over 12, handling conditions requiring second- and third-level care. France Diplomatie also lists, among the public facilities useful in an emergency, the Hospital San Rafael in Santa Tecla, the Hospital Bloom (paediatric), the Hospital Central and a gynaecological and paediatric hospital in San Salvador.

The Instituto Salvadoreño del Seguro Social (ISSS) manages mandatory affiliation for every employee in formal employment, with an employee contribution of 3% of salary (capped at 30 USD per month, on a maximum contribution base of 1,000 USD) and an employer contribution of 7.5% (capped at 75 USD per month). Unlike Nicaragua's INSS, the ISSS runs its own hospital network, including the Hospital Médico Quirúrgico in San Salvador, which concentrates specialised services (cardiac catheterisation, cardiovascular surgery, kidney transplant, oncology). Self-employed workers and legally resident foreigners who do not depend on a local employer can voluntarily join, between ages 18 and 60, the ISSS's special regime for self-employed workers, for a flat contribution of around 40 USD per month for individual cover or 56 USD for family cover (specialised sources; amounts and the time before cover takes effect to confirm directly with the ISSS).

San Salvador's private sector has two facilities that France Diplomatie lists among its useful contacts in an emergency: the Hospital de Diagnóstico (two sites, colonias Escalón and Médica), the country's largest private network by size, and the Centro Médico Escalón (formerly Hospital de la Mujer), which combines hospital care, diagnostic imaging and laboratory services. France Diplomatie recommends favouring the private sector when possible and notes that international insurance or a Visa card is in practice requested for payment. No vaccination is in principle mandatory for a stay from France outside a yellow fever booster for travellers coming from an endemic area, but hepatitis A and B, typhoid fever, rabies, tuberculosis for children under 15, and enhanced protection against chikungunya and dengue (mosquito-borne, circulating particularly during the rainy season from June to November) are recommended depending on planned exposure; France Diplomatie also reports recently detected malaria cases (late 2025) in certain tourist areas (coast, Route des Fleurs, Chalatenango department).

Legal and visa obligations

  • Tourist stay (up to 90 days for a French passport): no visa is legally required on entry. France Diplomatie presents insurance covering medical costs, hospitalisation and medical repatriation as essential in practice, without it being a legal entry requirement, as the embassy cannot cover private-sector medical costs.
  • CA-4 free movement area: El Salvador, Guatemala, Honduras and Nicaragua have created a movement area with no systematic control at shared land borders. France Diplomatie specifies, specifically for El Salvador, that the 90-day stay period applies across all four CA-4 countries, this counter therefore being cumulative and not reset when moving from one CA-4 country to another.
  • Extension beyond 90 days: a stay extension can be requested for a serious reason with Salvadoran immigration authorities, by putting together a file and paying a 25 USD fee, according to France Diplomatie.
  • Long-term move: France Diplomatie strongly recommends obtaining an immigration visa from the Salvadoran embassy in Paris before departure, while noting that Salvadoran authorities sometimes regularise a situation after arrival.
  • Rentista residency (passive income of around 1,460 to 2,000 USD per month according to the sources consulted, from dividends, rent, royalties or annuities) and pensionado residency (pension of around 1,200 to 1,500 USD per month, 1,224 USD according to Consortium Legal): one-year renewable temporary residency with the Dirección General de Migración y Extranjería (DGME), on a sworn declaration and proof of income. No documented requirement to take out health insurance was identified in the texts consulted for these two statuses, which in return prohibit any paid local work outside specialised state employment or teaching.
  • Digital nomad residency (visa launched in April 2025, foreign income of at least 1,460 USD per month for a sole applicant, higher with dependants): several specialised guides consulted indicate that comprehensive international health insurance, valid in El Salvador for the entire stay, is required when filing the application, a simple travel insurance policy not being enough; residency granted for one year, renewable up to four years, with no right to work for a local employer or clientele.
  • Seconded employees and employees in formal local employment : automatic ISSS affiliation upon hiring, with an employee contribution of 3% (capped at 30 USD/month) and an employer contribution of 7.5% (capped at 75 USD/month).

No official quantified fine was identified for simply lacking health cover outside the statuses mentioned above (notably the digital nomad visa); the documented risk remains full billing of care in the private sector, where a payment guarantee (international insurance or Visa card) is in practice requested on admission.

Social security agreements with France

No social security agreement has been signed between France and El Salvador: the country does not appear in CLEISS's list of country pages, which only covers countries that have concluded a coordination agreement with France.

  • In the absence of a bilateral agreement, no totalisation of insurance periods between the two countries is possible for opening pension rights.
  • No coordination mechanism allows even partial cover of care received in El Salvador under a French affiliation, nor the reverse for care received in France under an ISSS affiliation.
  • ISSS affiliation, whether resulting from formal local employment or the special regime open to self-employed workers and resident foreigners, provides local cover but does not substitute for any French social protection and is not recognised as such upon return to France.

For lack of an agreement, for any French national settling in El Salvador — an ISSS-affiliated employee, rentista/pensionado resident, digital nomad or retiree — voluntary membership of the Caisse des Français de l’étranger (CFE) or taking out international health insurance remain the only solutions for cover recognised in France, alongside any local cover.

Practical steps

  • Before departure: take out insurance covering medical costs, hospitalisation and medical repatriation, whatever the planned length of stay, as the Salvadoran private sector in practice requests a payment guarantee on admission.
  • Before travelling: check that yellow fever vaccination is up to date if coming from an endemic area, and find out about prevention measures against malaria and mosquito-borne diseases depending on the area visited.
  • For a stay planned across several CA-4 countries (El Salvador, Guatemala, Honduras, Nicaragua): account for the cumulative 90-day stay count before planning a sequence of countries.
  • For a long-term move: request an immigration visa from the Salvadoran embassy in Paris before departure, as recommended by France Diplomatie.
  • For a rentista or pensionado residency application: gather the required proof of income or pension and plan for health cover (ISSS special regime or private insurance) in the absence of a documented legal requirement.
  • For a digital nomad residency: gather proof of foreign income (from 1,460 USD/month) and take out, before filing the application, international health insurance valid in El Salvador for the entire stay.
  • Upon formal hiring in El Salvador: check with the employer that ISSS affiliation happens.
  • In the months following arrival: register with the roll of French nationals living abroad at the embassy in San Salvador (free, valid 5 years, recommended).
  • In the absence of a bilateral agreement: voluntary CFE membership or taking out international health insurance as soon as you settle.

Frequently asked questions

Is health insurance mandatory to enter El Salvador as a tourist?

No, none of the sources consulted require proof of insurance as a legal entry requirement for a visa-free stay (up to 90 days for a French passport). France Diplomatie nonetheless presents it as essential in practice, as the embassy cannot cover private-sector medical costs.

What is the CA-4 area and how does it affect a French tourist's length of stay in El Salvador?

CA-4 is a free movement area between El Salvador, Guatemala, Honduras and Nicaragua. France Diplomatie specifies, specifically for El Salvador, that the 90-day stay period applies across all four countries: this counter is cumulative and is not reset when moving from one CA-4 country to another.

Have France and El Salvador signed a social security agreement?

No. El Salvador is not among the countries with a social security coordination agreement with France listed by CLEISS. No totalisation of insurance periods nor cross-cover of care is therefore provided for between the two countries.

Does a resident under rentista or pensionado status need to affiliate to the ISSS or take out health insurance?

None of the sources consulted require this affiliation or taking out health insurance as a condition for obtaining these residency statuses (passive income of around 1,460 to 2,000 USD per month for rentista, pension of around 1,200 to 1,500 USD per month for pensionado, depending on the source). Foreign residents can nonetheless voluntarily join the ISSS's special regime for self-employed workers, though this local cover does not replace French social protection.

Does El Salvador's digital nomad residency require health insurance?

Several specialised guides consulted indicate that comprehensive international health insurance, valid in El Salvador for the entire stay, is required when filing an application for this status (minimum foreign income of 1,460 USD per month for a sole applicant), a simple travel insurance policy not being enough. No minimum cover amount could be identified in a published official text; it should be confirmed directly with Salvadoran immigration authorities before any application.
