Healthcare Reimbursement in the Åland Islands: Who Reimburses What Depending on Your Situation

Key points

In the Åland Islands, the carte Vitale is not used at the doctor's: the European regulations organise coordination, and it is status that determines the payer. Public care is delivered by the ÅHS, whose fees are set by Åland legislation, while Kela, a national body, reimburses part of medicines and private care and manages certain benefits in the archipelago. The tourist or student on a temporary stay presents the EHIC, the seconded employee keeps the French scheme with form A1, the retiree hands their S1 to the competent Finnish body, and the local employee comes under the Finnish system. A member of the CFE (the Caisse des Français de l'Étranger, the voluntary health insurance scheme for French expatriates) or someone with first-euro insurance completes this base according to their contract. The ÅHS amounts could not be read: they are in its fee manual.

  • 6 months: deadline for asking Kela for partial reimbursement of private care or a medicine advanced by a visitor with their EHIC (CLEISS, the French centre for European and international social security liaison).
  • €800.02 a month: minimum employment income opening certain Kela benefits from the start of employment in Finland (Kela).
  • €8 per consultation with a private doctor: Kela's national reimbursement scale, whose application in Åland should be confirmed (Kela).
  • €70.33 annual deductible and €636.12 annual ceiling for reimbursed medicines (Kela, national scale).
  • 3.2% on the basic pension and 4.2% on the supplementary pension: health contribution withheld on the general scheme pension of a retiree whose tax residence is outside France (CLEISS).
  • Supplement for a weekend, public holiday or on-call dental visit at the ÅHS; other fees in the tariff manual in force.

The Principle: Status Determines the Payer

France and Finland being members of the European Union, their social security schemes are coordinated by the European regulations, and the CLEISS pages consulted provide no exception for Åland. Proof of entitlement is the EHIC for a temporary stay, or a portable form (A1, S1) for a lasting stay. The notions of reimbursement basis and out-of-pocket cost help compare cover.

Åland adds a particularity: three levels overlap. The ÅHS delivers public care and charges the fees set by the archipelago’s legislation (fee manual and rights to protection against high costs, whose amounts could not be read). Kela, a national body, reimburses part of medicines and private care and manages certain benefits; according to it, Åland’s social legislation differs in part from that of the mainland, and other benefits come under the AMS, the KST or the ÅHS. Lastly, the European regulations designate the competent country. How the care network works is described in the page healthcare system in the Åland Islands.

Tourist and Temporary Stay: the EHIC

According to the ÅHS, all patients are entitled to urgent public sector care. Residents of the EU, the EEA, the United Kingdom and Switzerland pay the normal rate on presentation of the EHIC, valid identification and their home address; without the EHIC or another document proving entitlement to care, they pay the full cost. CLEISS indicates that if the card is forgotten, lost or stolen, a provisional certificate valid for three months can be issued.

  • Private care: the patient pays up front, then asks Kela for partial reimbursement, with invoices, EHIC and identification, within six months; Kela does not reimburse public sector care (CLEISS).
  • Without an EHIC: the patient can ask their French fund for reimbursement on return, with form S3125 (CLEISS).
  • Planned care: the ÅHS applies the Cross-Border Healthcare Act; without an S2 certificate, the patient bears the cost.
  • Assistance and repatriation: France Diplomatie (the French foreign ministry) reminds readers that it is essential to hold an assistance contract or insurance covering medical costs, hospitalisation and repatriation. See long-stay travel insurance.

Local-Contract Employee, Self-Employed and Person with No Activity

The local-contract employee comes under Finnish legislation, social insurance being a State power. Kela indicates that a worker who earns at least €800.02 a month in Finland obtains certain benefits from the start of employment, such as sickness allowance and the Kela card, whatever their origin. For public care, they pay the ÅHS fees; as Kela does not reimburse the public sector, it intervenes for medicines and private care, subject to confirmation of their application in Åland. See the glossary entry on the local contract.

The self-employed person comes under the Finnish system depending on their residence or activity; carrying on their activity in Åland also requires the hembygdsrätt, five years of residence or a right to carry on an activity (näringsrätt), as the page working in the Åland Islands details.

A person with no activity who settles for more than three months shows sufficient resources for their right of residence (InfoFinland). The pages consulted do not specify access to the ÅHS for a resident with no municipality of residence or affiliation: this point should be confirmed with the ÅHS (kund.ekonomi@ahs.ax) and Kela, and bridging cover is useful during the procedures.

Seconded Employee, Retiree, Student and Family

The seconded employee of a French employer remains attached to French Sécurité sociale with form A1. For their care, CLEISS indicates requesting from their French fund an S1 form to hand to the local body, or being reimbursed by the French fund, the EHIC giving direct access to care in the country of secondment. A sick note is sent to the French fund within 48 hours, and form S3125 allows a claim for reimbursement of costs paid up front. See the seconded employee glossary entry and the guide seconded or expatriated.

The retiree who receives only a French pension and does not work requests form S1 from their pension fund before departure, then hands it to the health body of the country of residence (CLEISS). If their tax residence is outside France, a health contribution of 3.2% on the basic pension and 4.2% on the supplementary pension is withheld from general scheme pensions; CSG and CRDS do not apply. The handling of the S1 in the archipelago should be confirmed with Kela and the ÅHS. See the guide form S1: how to use it and the page retiring in the Åland Islands.

The student on a temporary stay uses the EHIC. For a stay of more than three months, the right of residence is registered on the basis of a place as a student (InfoFinland). Kela specifies that financial aid for studies is addressed, at the student’s choice, to Kela or the AMS for a person who lived permanently in Åland before their studies. See student insurance abroad.

The spouse and children access care according to their own situation of residence and the status of the insured person (CLEISS). See expat family insurance.

CFE Member, First-Euro Insurance and Return to France

The CFE member keeps French cover by contributing to both systems: according to ameli.fr, they pay the compulsory contributions of the country of expatriation and those of the CFE, which avoids waiting periods on return and lost quarters for retirement. The conditions of membership and the waiting periods should be checked with the CFE. See the guide CFE reimbursements.

International health insurance with first-euro cover reimburses actual costs within the limit of its ceilings, after any deductible, waiting periods and exclusions. It is useful for private care, which Kela reimburses only in part, for assistance and repatriation, and to cover the period before access to local entitlements.

The return to France: according to ameli.fr, the expatriate who comes back notifies their departure to the health insurance body of the country they are leaving, then files form S1106 requesting the opening of rights with their French fund. See the guide reducing the waiting period and the page return to France.

Item by Item: Consultation, Hospital Stay, Medicines, Dental, Optical, Maternity

  • Consultation: at the ÅHS health centre, without a referral letter, at the fees in the ÅHS manual; with a private doctor, Kela reimburses €8 per consultation under its national scale, with higher amounts for certain specialities (Kela).
  • Hospital stay and emergencies: ÅHS public care at the fees in its manual; an EU visitor with no EHIC pays the full cost. Journeys to a care provider in Åland can be reimbursed by Kela; to mainland Finland or Sweden, only with an ÅHS voucher.
  • Medicines: Kela reimburses prescribed medicines; annual deductible and annual ceiling of out-of-pocket cost revised every year (national scale).
  • Dental: ÅHS public care for children and young people up to age 28 and priority groups; adults turn to their usual dentist. Dental emergencies are open to visitors, with a €50 supplement at weekends, on public holidays or on call.
  • Optical: the official pages consulted describe no cover of glasses for adults; international contracts set a ceiling per period.
  • Maternity: the ÅHS offers advice to pregnant women, screening, birth preparation, maternity and delivery; the birth and adoption allowance is paid by the KST (Kela, CLEISS).

The reimbursement guide and the page expat health insurance give benchmarks; Expavy helps you weigh these options against your situation.

Frequently asked questions

Does the carte Vitale work in the Åland Islands?

No. For a temporary stay, the EHIC opens the normal rate for ÅHS urgent care, with valid identification and the home address. For a lasting stay, proof of entitlement depends on status: local affiliation, form A1 for a seconded employee or form S1 for a retiree.

How much does ÅHS care cost?

Fees are set by Åland legislation and published in the ÅHS fee manual. The amounts could not be read in the sources consulted; the ÅHS cashier service (018-535 163) provides them. A weekend, public holiday or on-call dental visit costs €50 more.

What happens without an EHIC in the Åland Islands?

The ÅHS treats emergencies, but the patient pays the full cost of treatment. They can ask their French fund for reimbursement on return with form S3125.

Can a French retiree be treated in the Åland Islands?

Yes, with form S1 requested from their pension fund before departure and handed to the local body; a health contribution of 3.2% and 4.2% is withheld from their French pensions if their tax residence is outside France. The handling of the S1 in Åland should be confirmed with Kela and the ÅHS.

Does Kela reimburse private care in the Åland Islands?

Kela states that it reimburses €8 per private doctor consultation, with higher amounts for certain specialities, but its pages do not mention Åland: the application in the archipelago should be confirmed with it. It does not reimburse public sector care.

How is a seconded employee covered in the Åland Islands?

They stay in the French scheme with form A1. For their care, they request an S1 from their French fund or are reimbursed by it; costs paid up front are reimbursed with form S3125 and sick notes are declared within 48 hours.

How do you reopen your rights on returning to France?

By notifying your departure to the local health insurance body, then filing form S1106 requesting the opening of rights with your French health insurance fund (ameli.fr). A CFE member checks the conditions for resuming with the body.
