Expat insurance and health insurance in Turkmenistan

In short
As France and Turkmenistan are not linked by any bilateral social security agreement, there is neither a European Health Insurance Card (Turkmenistan is neither an EU nor an EEA member) nor any coordination form applicable on the ground. A visa is mandatory for any stay, with an invitation letter issued by the State Migration Service that can take three to four weeks to obtain, and registration with the Migration Service is required within three days of arrival for any extended stay. France Diplomatie describes local medical infrastructure as insufficient and states that all hospitalisation costs — care, accommodation, linen, medicines and meals — remain at the foreign patient's expense, hence the importance of comprehensive health and repatriation insurance taken out before departure, with many international policies also excluding Turkmenistan from their coverage. The French embassy in Ashgabat has no consular section, with consular matters falling to the French embassy in Baku, Azerbaijan. An employee sent to Turkmenistan by a French employer can remain affiliated to the French scheme through unilateral secondment under French law, in the absence of an agreement.
Key figures
- Life expectancy at birth: 70.2 years in 2024 (World Bank).
- Current health spending: 5.50% of GDP in 2023, around 581 dollars per capita (World Bank).
- Mandatory visa for any stay, with an invitation letter taking three to four weeks to obtain (France Diplomatie).
- Mandatory registration with the Migration Service within 3 days of arrival, beyond 3 working days of stay (France Diplomatie).
Local healthcare system
France Diplomatie describes insufficient medical infrastructure in Turkmenistan. In case of serious illness or accident, medical evacuation is presented as necessary. All costs incurred at a Turkmen facility — care, accommodation, linen, medicines and meals — remain at the foreign patient's expense, with no cover from a local scheme accessible to French nationals passing through.
Comprehensive health and repatriation insurance is presented by France Diplomatie as essential before any departure. Many international policies exclude Turkmenistan from their coverage area, which makes it necessary to carefully check the terms of the policy taken out. The French embassy can never cover medical costs, even in an emergency.
On the health front, France Diplomatie recommends, in addition to universal vaccines (diphtheria, tetanus, polio, whooping cough, measles), vaccination against hepatitis A and B, typhoid fever, as well as, depending on the length and nature of the stay, tuberculosis (children under 15, extended stays) and rabies (particularly for young children or in case of a stay in a rural area). Typhoid fever (contaminated water and food) and rabies (contact with infected animals) are among the identified health risks, hence strict recommended food hygiene rules (bottled or boiled water, well-cooked food, raw vegetables, shellfish and reheated dishes to avoid, frequent hand washing).
The country is also placed under heightened vigilance by France Diplomatie, which formally advises against all travel in the border area with Afghanistan and notes a seismic risk across the whole territory — two factors that reinforce the value of cover including medical repatriation.
Legal and visa obligations
- Mandatory visa for any stay, to apply for with the Turkmenistan embassy in Paris before departure, or to obtain at Ashgabat airport or Turkmenbashi port; passport valid at least 6 months after the planned return date.
- Mandatory invitation letter, issued by the State Migration Service at the request of the inviting party (travel agency, company or Turkmen private individual), which can take three to four weeks to obtain.
- Entry taxes: an additional tax of 12 US dollars and a tax of 2 US dollars per hotel night, not stated as exhaustive by France Diplomatie.
- Health test on arrival: a paid antigen test, billed at 50 US dollars, is noted by France Diplomatie.
- Mandatory registration with the Migration Service within three days of arrival, for any stay exceeding three working days; failure to comply can lead to a fine, deportation with a one- to several-year entry ban, or even imprisonment.
- No European Health Insurance Card: as Turkmenistan is neither an EU nor an EEA member, the EHIC does not apply there, regardless of the length of stay.
- Health and repatriation insurance: presented as essential by France Diplomatie, as no cover of medical or repatriation costs is provided by the French embassy, and many international policies exclude the country from their coverage area.
- Seconded employee : in the absence of a France-Turkmenistan agreement, remaining affiliated to the French scheme relies solely on French law (unilateral secondment), for a period of 3 years renewable once, with no exemption from any contributions owed in Turkmenistan.
Without health insurance cover, care is billed directly to the patient by the facility, rate varying by facility and not publicly disclosed; the risk remains primarily financial, with no assistance from the French embassy for medical costs, which moreover has no consular section in Ashgabat.

Social security agreements with France
France and Turkmenistan are not linked by any bilateral social security agreement: CLEISS lists Turkmenistan neither among the countries where EU coordination regulations apply, nor among those having concluded a bilateral agreement with France, and no Central Asian country appears on its list of agreements in force. There is therefore no European Health Insurance Card, no A1 or S1 form enforceable with the Turkmen authorities, and no direct reimbursement mechanism between the two systems.
- Temporary stay: no European or bilateral coordination applies; only health/repatriation insurance taken out before departure covers costs on the ground, with France Diplomatie presenting this cover as essential.
- Secondment from France: absent an agreement, French law provides for a unilateral secondment of 3 years renewable once. The employee remains affiliated to the French scheme, with contributions paid in France, but with no exemption from any contributions owed in Turkmenistan. Care received on the ground is reimbursed afterwards by the French fund, within the limit of French rates, on presentation of receipts and form S3125.
- Long-term move: no local health scheme is automatically accessible to foreign residents, and no transfer of rights from France is provided for. Voluntary CFE membership remains possible.
- Retiree: no coordination mechanism allows rights acquired in France to be claimed; international health insurance remains necessary for any extended stay.

| Student | No EHIC (Turkmenistan outside EU/EEA); no accessible local health scheme affiliation | Student or comprehensive travel insurance to take out before departure, for the whole stay, checking geographic coverage |
| Seconded employee | French unilateral secondment (3 years renewable once), with no exemption from any Turkmen contributions | Reimbursement of local care limited to French rates (form S3125); supplementary cover recommended given medical infrastructure described as insufficient |

Frequently asked questions
Is there a social security agreement between France and Turkmenistan?
No. No bilateral social security agreement links France and Turkmenistan, and no Central Asian country appears on the list of agreements in force published by CLEISS: neither the European Health Insurance Card nor A1 or S1 forms apply.
Does a French citizen need a visa for Turkmenistan?
Yes, a visa is mandatory for any stay, whatever its length. It is applied for with the Turkmenistan embassy in Paris before departure, or obtained at Ashgabat airport or Turkmenbashi port, and requires an invitation letter issued by the State Migration Service, which can take three to four weeks to obtain.
Does the European Health Insurance Card (EHIC) work in Turkmenistan?
No. Turkmenistan is neither an EU nor an EEA member, so the EHIC does not apply there, regardless of the length of stay.
Is health insurance essential to travel to Turkmenistan?
France Diplomatie presents comprehensive health and repatriation insurance as essential before any departure, as local medical infrastructure is judged insufficient and all hospitalisation costs remain at the foreign patient's expense. Many international policies also exclude Turkmenistan from their coverage area, which should be checked before departure.
Can the Turkmen health system treat all conditions?
France Diplomatie describes insufficient medical infrastructure and states that medical evacuation is necessary in case of serious illness or accident. Cover including repatriation abroad is therefore recommended.
How is an employee seconded from France covered in Turkmenistan?
Absent an agreement, they can benefit from unilateral secondment under French law (up to 3 years renewable once). They remain affiliated to the French scheme, with care reimbursed afterwards on presentation of form S3125, but with no exemption from any contributions owed in Turkmenistan.
Can the French embassy in Turkmenistan help in case of a medical problem?
The French embassy in Ashgabat has no consular section: consular matters fall to the French embassy in Baku, Azerbaijan. France Diplomatie further states that no cover of medical costs is possible from the French mission, even in an emergency, hence the importance of insurance taken out before departure.
