Healthcare Reimbursement in Belize: Who Reimburses What Depending on Your Situation
Key points
In Belize, the carte Vitale does not work: France has no social security agreement with this country, and it is your status that determines who pays. The locally employed worker contributes to the Social Security Board, which pays cash benefits without reimbursing care; national health insurance covers only certain areas and primary care; the seconded employee in principle keeps the French scheme under the conditions of the secondment. The retiree, the self-employed person and the digital nomad choose between the CFE (the French expatriates' health fund) and international insurance, and the Work Where You Vacation programme requires travel insurance of at least USD 50,000. The summary table sets out each case, then the page details the care items.
- No France-Belize social security agreement: CLEISS, the French liaison centre for international social security, lists none.
- Medical insurance not required for a tourist stay but strongly recommended; without it access to care may be refused, even in a life-threatening emergency (France Diplomatie).
- Work Where You Vacation: travel insurance with at least USD 50,000 of cover required (Belize Tourism Board).
- CFE: contributions that depend on age and the plan chosen, for the main offer, the youth offer, the retirees' offer; no waiting period if enrolment takes place before departure or within three months.
- Health expenditure paid directly by households: around 23% of total health expenditure (PAHO).
- Return to France: free three-month retention of rights for a CFE member with no activity, then PUMa, the French universal health protection scheme (Assurance Maladie).
Service being launched: an advisor reviews your request and gets back to you to clarify your needs.
The Principle: Four Possible Players, Nothing Automatic
France Diplomatie (the French foreign ministry) specifies that an assistance contract or insurance covering all medical costs, surgery, hospitalisation and repatriation is essential, and that the French embassy cannot pay for them. CLEISS lists no social security agreement with Belize: no coordination between schemes, no totalisation of periods, no cross-border payment of care (see the bilateral agreement).
Four players therefore come into play: the local scheme (public network, national health insurance and Social Security Board), the French scheme kept in place (secondment), the CFE and an international health insurance. The notions of reimbursement base and out-of-pocket cost help you compare. How the network is organised is presented in the page on the Belize healthcare system.
Locally Employed Worker and Seconded Employee
The locally employed worker is registered by their employer with the Social Security Board, which pays allowances for sickness, maternity and work accidents, and pensions: these cash payments do not reimburse medical costs. Care is received in the public network, whose user fees were abolished according to PAHO with no date given, or in the private sector, which is paid. National health insurance operates only in certain areas, on primary care. See the local contract.
The seconded employee in principle remains attached to French social security during the assignment. With no agreement with Belize, this retention falls under French law on secondments outside an agreement: duration, conditions and contributions should be checked with Urssaf (the French social security contributions collector) and CLEISS before departure. Costs are reimbursed under French rules, on paid invoices, whatever price is charged locally; employers often add an international group contract. See seconded or expatriate and the glossary entry on the seconded employee.
Self-Employed, Digital Nomad, Student, Spouse and Children
The self-employed person has no employer to affiliate them: they combine the public sector, the CFE and international insurance. The digital nomad enrolled in the Belize Tourism Board’s Work Where You Vacation programme must show travel insurance with at least USD 50,000 of cover, in addition to annual income of USD 75,000 (USD 100,000 for a couple or a family); the travel insurance required is a minimum, to be compared with international health cover for repeated stays. See digital nomad insurance.
The student needs a student permit issued by the immigration department; before age 30, the CFE offers its youth offer. See student insurance abroad. The spouse and children appear in residence files (only a legally married spouse can be included) and come under CFE family contracts or international ones; see expat family insurance.
Retiree: QRP Programme and No S1 Form
The S1 form is valid only in the European Union and equivalent States: a retiree settled in Belize does not keep automatic French cover. The Belize Tourism Board’s qualified retirees programme (QRP) mentions no health insurance requirement on the page consulted: it requires a medical certificate less than three months old, but cover remains to be arranged. The retiree chooses between the public sector, the CFE (retirees’ offer, with a contribution that depends on age and the plan chosen according to cfe.fr) and international insurance. See the guide to health insurance for retirees abroad and the page on retiring in Belize.
CFE Member: Reimbursement Abroad, Waiting Period and Return to France
The CFE is a voluntary social protection body. In France, it reimburses according to the principles of social security, with direct payment (third-party payment) in public and private hospitals, for stays of less than three months; abroad, the amounts depend on the country of care and the procedure, in the form of rates or flat amounts applied to actual expenses. Example from cfe.fr: a GP consultation billed €25 in France is reimbursed €21 by social security, and the CFE reimburses at most €21 abroad. When local prices exceed the base used, the difference remains payable by the member (see CFE reimbursements).
- Offers: the main offer (age 30 and over), the youth offer (under 30) and the retirees’ offer, each with a contribution that depends on age and the plan chosen, and an offer for care in France only.
- Waiting period: immediate cover if enrolment takes place before departure or within three months of settling; otherwise three months, or six months from age 45 (see waiting period).
- Maternity: conception must come after enrolment or after attachment as a dependant.
On returning to France, a CFE member with no activity benefits free of charge from a retention of rights of three months (Assurance Maladie, the French national health insurance), then joins the CPAM (the local health insurance office) under PUMa; without activity, three months of uninterrupted residence are required, a condition from which the holder of a French old-age pension is exempt. See reducing the waiting period and returning to France.
First-Euro Insurance and the Tourist
International health insurance with first-euro cover reimburses actual costs within its ceilings, after any deductible, waiting periods and exclusions. It opens access to the private sector with direct payment and finances evacuation to the United States, Mexico or the Caribbean mentioned by France Diplomatie for serious cases. Compare the annual ceiling, the zone of cover, evacuation and the treatment of pre-existing medical conditions.
The tourist stays less than 30 days without a visa. Medical insurance is not required on entry but is strongly recommended by France Diplomatie; the assistance included with a bank card can supplement it, with ceilings to be checked. See long-stay travel insurance.
Item by Item: Consultation, Hospitalisation, Medicines, Dental, Optical, Maternity
- Consultation: public sector under the ministry’s conditions; private sector paid by the patient unless covered by a contract; reimbursed by the CFE according to its country scale; actual costs under a first-euro contract.
- Hospitalisation: French scheme for the seconded employee; CFE with third-party payment in France; international contract with direct payment; the reference hospital offers payment facilities but does not publish its fees.
- Medicines: paid by the patient outside a contract; reimbursed according to CFE scales or within the ceilings of an international contract.
- Dental: included in the CFE according to its scales and in most international contracts with an annual ceiling.
- Optical: mainly a matter for international contracts, with ceilings per period.
- Maternity: Social Security Board cash benefits for contributors; the CFE requires conception after enrolment; international contracts generally apply a waiting period.
The reimbursement guide and the page on expat health insurance give benchmarks; Expavy helps you compare these options against your situation.
