Expat insurance and health insurance in Belize

Belize's health system is organised by the Ministry of Health and Wellness, which runs a network of regional public hospitals: the Karl Heusner Memorial Hospital, in Belize City, serves both as the national reference hospital and as a district hospital, and coordinates complex cases with the regional hospitals of Belmopan (Western Regional Hospital), Orange Walk (Northern Regional Hospital), Stann Creek/Dangriga (Southern Regional Hospital), Corozal, San Ignacio and Punta Gorda. France Diplomatie describes the country's medical infrastructure as underdeveloped, particularly outside Belize City and Belmopan, and indicates that serious cases often require evacuation to the United States, Mexico or the Caribbean islands.

The Social Security Board (SSB) is Belize's social security body, funded by contributions capped at an insurable salary of 540 BZD per week, at an overall rate of 10%, split into eight salary brackets where the employee share ranges from 1.88% to 4.50% and the employer share from 5.50% to 8.13%. The SSB pays short-term benefits (sickness benefit, maternity allowance and grant, funeral grant), occupational injury benefits and long-term pensions, but does not directly cover medical or hospital costs: these are cash benefits paid to the contributing employee, not a health insurance scheme in the sense of the Honduran IHSS or the Guatemalan IGSS. A person who has stopped working can apply, within 26 weeks of the end of their employment and subject to at least 150 contributions already paid, for voluntary contributor status (3.2% of average insurable salary) to keep building rights to long-term benefits; this continuation mechanism is not open to a foreigner newly arrived with no contribution history in Belize.

Separate from the SSB, a National Health Insurance (NHI) scheme, funded from general revenue and administered by the Social Security Board, funds primary health care (general medicine, ophthalmology, management of diabetes and hypertension) in public polyclinics, but its geographic coverage remains, according to information published by the SSB, limited to the southern part of Belize District (pilot zone since 2001) and to the Southern region of the country (rolled out since 2006), with an extension to Orange Walk district underway more recently; the NHI therefore does not yet cover the whole of Belizean territory. Consultations at public facilities remain of nominal cost for anyone presenting there, in the range of a few Belize dollars according to specialised guides, with laboratory and radiology tests remaining, however, at the patient's expense.

The private sector is concentrated in Belize City, with Belize Healthcare Partners and Belize Medical Associates, as well as the La Loma Luz Hospital, in Santa Elena (Cayo). France Diplomatie indicates that insurance covering medical costs, surgery, hospitalisation and medical evacuation is essential, as the embassy cannot cover these costs under any circumstances. Universal French vaccinations must be up to date, and yellow fever, hepatitis A and B, typhoid fever and rabies are recommended depending on expected exposure. Dengue is on the rise in the country according to France Diplomatie, zika and chikungunya also present a risk, while malaria remains confined to specific southern areas (Stann Creek district), not affecting Belize City or the tourist islands.

• Tourist stay (up to 30 days for a French passport): no visa is required on entry, and the passport must remain valid for at least 6 months after arrival. A return ticket or onward ticket to another destination must be shown. France Diplomatie specifies that health insurance is no longer a legal condition of entry but remains strongly recommended; visitors must also stay in accommodation approved by the authorities ("Gold Standard Hotels") and show their booking confirmation on arrival at the airport.
• Extending the tourist stay: the Visitor's Permit can be extended 30 days at a time at any Immigration Department office, for 200 BZD (around 100 USD) per extension, with no limit on renewals mentioned by the official source consulted; the holder of this permit may not carry out any paid, professional or commercial activity without express authorisation from the Director of Immigration.
• Long-term stay / settling: beyond the extended tourist regime, a Long Stay Permit or another residency category must be requested from the Belizean Immigration Department; France Diplomatie does not detail the conditions specific to each residency category and refers to the Belizean migration authorities.
• Qualified Retired Persons (QRP) residency, governed by the Retired Persons (Incentives) Act as amended in 2023: minimum age of 40 (lowered from 45), permanent income of foreign origin of at least 2,000 USD per month (24,000 USD per year), medical certificate of absence of communicable disease and a police record from the country of residence. No documented obligation to take out health insurance or affiliate with the SSB has been identified in the sources consulted for this status, even though specialised guides emphasise the limits of the public system outside Belize City.
• "Work Where You Vacation" digital nomad visa (Belize Tourism Board): foreign income of at least 75,000 USD per year for a single applicant (100,000 USD for a couple or family), notarised bank statement, police record less than 6 months old, and a health/travel insurance policy with at least 50,000 USD of cover explicitly required for the entire length of stay. The status is granted for 6 months, extendable, for a fee of 500 BZD (250 USD) per adult and 200 BZD (100 USD) per dependent child.
• Seconded employees and employees in formal local employment : affiliation with the Social Security Board from the date of hiring, overall contribution of 10% of insurable salary capped at 540 BZD per week, split across eight brackets between the employee share (1.88% to 4.50%) and the employer share (5.50% to 8.13%); these contributions open rights to cash benefits (sickness, maternity, occupational injury, retirement) but do not directly fund access to care.

Security, addressed here only from the angle of access to care and assistance: France Diplomatie places the whole of Belize under heightened vigilance and reports declining security in Belize City, Dangriga, the border areas with Guatemala, as well as San Pedro and Ambergris Caye, due to rising crime; driving at night is advised against and hurricane season (June to November) presents a natural disaster risk in its own right. This geographic distribution in practice shapes access to health facilities, most of which are located in Belize City.

No official monetary fine has been identified for a simple lack of health cover outside the statuses mentioned above; the documented risk remains full billing of care and additional tests (laboratory, imaging) in both the public and private sectors.

No social security agreement has been signed between France and Belize: the country does not appear on the CLEISS list of country fact sheets, which only covers countries that have concluded a coordination agreement with France.

• In the absence of a bilateral agreement, no totalisation of insurance periods between the two countries is possible for establishing retirement rights.
• No coordination mechanism allows for even partial coverage of care received in Belize under a French affiliation, nor the reverse for care received in France under affiliation with the Belizean Social Security Board.
• Affiliation with the SSB, resulting from formal local employment, opens rights to local cash benefits (sickness, maternity, occupational injury, retirement) but does not directly fund access to care and does not substitute for any French social protection; it is not recognised as such upon return to France.

In the absence of an agreement, for any French national settling in Belize — an employee affiliated with the SSB, a QRP resident, a digital nomad or a retiree — voluntary membership of the Caisse des Français de l’étranger (CFE) or taking out international health insurance remain the only solutions for having cover recognised in France, in addition to any local cover.

• Before departure: take out insurance covering medical costs, surgery, hospitalisation and medical repatriation, regardless of the length of stay planned.
• Before travelling: check that the passport remains valid for at least 6 months after arrival and gather proof of a return ticket or onward ticket to another destination.
• Before any travel outside Belize City or Belmopan: check France Diplomatie's travel advice, as the whole country is classified under heightened vigilance.
• To extend a tourist stay beyond 30 days: go to an Immigration Department office before the current permit expires and pay the 200 BZD extension fee.
• For a QRP residency application: gather the medical certificate, police record and proof of permanent income (minimum 2,000 USD/month), and plan for private health cover in the absence of a documented legal obligation.
• For a digital nomad visa application: take out health/travel insurance with at least 50,000 USD of cover beforehand and gather the notarised bank statement required by the Belize Tourism Board.
• From the moment of formal hiring in Belize: check with the employer that affiliation with the Social Security Board is in place.
• In the months following arrival: register with the roll of French nationals living abroad at the consular agency in Belize City, attached to the French embassy in Guatemala (free, valid 5 years, recommended).
• In the absence of a bilateral agreement: voluntary membership of the CFE or taking out international health insurance from the moment of settling.
