Expat insurance and health insurance in Tajikistan

France Diplomatie describes limited medical infrastructure in Tajikistan, with sterilisation systems often insufficient; it is recommended to consult the embassy before seeking individual care from a local hospital or practitioner. For any serious surgery, medical evacuation to a third country is recommended, as no local facility has advanced MRI or CT scan equipment. Two private facilities are listed in Dushanbe: the Prospekt clinic for general medical care and the Eurodent clinic for dental care.

Comprehensive health and repatriation insurance, covering all medical, surgical and hospitalisation costs as well as medical evacuation, is presented by France Diplomatie as essential before any departure: these costs can never be covered by the French embassy.

On the health front, France Diplomatie recommends, in addition to universal vaccines (diphtheria, tetanus, polio), vaccination against hepatitis A and B, typhoid fever, and rabies, particularly for young children in rural areas. Malaria is reported from June to October in certain regions, alongside typhoid fever (contaminated water and food) and the risk of rabies from contact with infected animals, hence strict recommended food hygiene rules.

The country is also placed under heightened vigilance by France Diplomatie across most of the territory, with a zone formally advised against along the Afghan border (Khatlon province) and mined sectors near the Uzbek border. Sustained seismic activity (magnitude 3 to 8 on the Richter scale) as well as avalanches, floods and landslides in mountainous areas are also reported — factors that reinforce the value of cover including medical repatriation.

• No visa required for a tourist stay not exceeding 30 days.
• Electronic visa (evisa.tj) beyond 30 days: valid for up to 90 days, for an actual stay limited to 60 days according to France Diplomatie; passport valid for at least 6 months after the application date.
• Special permit for Gorno-Badakhshan (GBAO): any visit to this region subject to authorisation, raising the electronic visa cost from 50 to 70 dollars.
• Mandatory registration within ten working days of arrival with the Ministry of the Interior (OVIR), except for electronic visa holders or stays under ten working days.
• No European Health Insurance Card: as Tajikistan is neither an EU nor an EEA member, the EHIC does not apply there, regardless of the length of stay.
• Health and repatriation insurance: no cover of medical, surgical or repatriation costs is provided by the French embassy; France Diplomatie recommends comprehensive cover before departure, given limited local medical infrastructure.
• Seconded employee : in the absence of a France-Tajikistan agreement, remaining affiliated to the French scheme relies solely on French law (unilateral secondment), for a period of 3 years renewable once, with no exemption from any contributions owed in Tajikistan.

Without health insurance cover, care is billed at full rate by the facility, public or private, rate varying by facility and not publicly disclosed; the risk remains primarily financial, with no assistance from the French embassy for medical, surgical or medical evacuation costs.

France and Tajikistan are not linked by any bilateral social security agreement: CLEISS lists Tajikistan neither among the countries where EU coordination regulations apply, nor among those having concluded a bilateral agreement with France, and no Central Asian country appears on its list of agreements in force. There is therefore no European Health Insurance Card, no A1 or S1 form enforceable with the Tajik authorities, and no direct reimbursement mechanism between the two systems.

• Temporary stay: no European or bilateral coordination applies; only health/repatriation insurance taken out before departure covers costs on the ground, with France Diplomatie recommending comprehensive cover.
• Secondment from France: absent an agreement, French law provides for a unilateral secondment of 3 years renewable once. The employee remains affiliated to the French scheme, with contributions paid in France, but with no exemption from any contributions owed in Tajikistan. Care received on the ground is reimbursed afterwards by the French fund, within the limit of French rates, on presentation of receipts and form S3125.
• Long-term move: no local health scheme is automatically accessible to foreign residents, and no transfer of rights from France is provided for. Voluntary CFE membership remains possible.
• Retiree: no coordination mechanism allows rights acquired in France to be claimed; international health insurance remains necessary for any extended stay.

1. Before departure: notify French agencies of your departure (CPAM, CAF, France Travail, pension funds).
2. Before departure: take out health/repatriation insurance covering all medical, surgical costs and medical evacuation, recommended by France Diplomatie given limited medical infrastructure.
3. Check passport validity (at least 6 months) before any visa application.
4. For a stay longer than 30 days: apply for an electronic visa via evisa.tj before departure.
5. For any travel to Gorno-Badakhshan (GBAO): apply for the required special permit in addition to the visa.
6. On arrival, unless exempt: register within ten working days with the Ministry of the Interior (OVIR).
7. Seconded employee: arrange unilateral secondment with the employer through the French fund before departure (maximum duration 3 years renewable once).
8. Keep care receipts and form S3125 for any subsequent reimbursement request with the French fund (secondment case).
9. Register with the roll of French nationals living abroad at the French embassy in Dushanbe.
