Medical questionnaire (questionnaire médical)
A form filled in when taking out a private health insurance policy, in which the policyholder declares their state of health; it forms the basis of the contract and the insurer's risk assessment.
The medical questionnaire is filled in by the policyholder when taking out private health insurance: it details medical history, current treatments, and general state of health. The insurer uses it to assess risk and decide whether to accept the contract, apply an exclusion or an additional premium, or decline it.
Article L113-2 of the French insurance code requires the policyholder to answer the questions asked accurately. An intentional false statement exposes the contract to being voided under Article L113-8: the insurer can then refuse any reimbursement, including for care unrelated to the omission, as soon as the intentional nature is established.
Example
Deliberately omitting to mention an ongoing treatment on the medical questionnaire, hoping to get a lower price, exposes the contract to being voided if the insurer discovers it later — even if the claim that occurs has no connection to the omitted information.
