Retirement abroad: what changes for health insurance
A retiree who settles abroad no longer has an employer to arrange their cover, and their French pension alone isn’t enough to guarantee access to care locally. Which mechanism applies depends first on the country of residence.
EU, EEA, Switzerland: form S1, and its absence elsewhere
In an EU or EEA country, or in Switzerland, a retiree receiving a French pension can, under certain conditions, apply for form S1 to be affiliated with the local public scheme with no new contribution — see in detail how form S1 actually works. That is the case for a retiree settling in Portugal. Outside this European coordination area, this mechanism simply doesn’t exist: the French pension remains income, not health cover, and access to care then depends on a bilateral agreement specific to the country (where one exists) or on private insurance. That is the case in Thailand, where health insurance is a condition of the retirement visa rather than a right opened up by the pension.
Why age and medical history weigh heavily on the price
Senior health insurance follows a different pricing logic than a policy for someone of working age: the price generally rises with age at the time of signing up, and a medical questionnaire can change the premium or exclude certain declared medical history. Some policies also apply an age cap at signup, beyond which membership is no longer possible. These rules vary from one insurer to another and need to be checked before treating a benefit as guaranteed, not discovered at the moment of a claim.

Whether form S1 applies or not, CFE remains worth knowing about: its RetraitExpat Santé offer is aimed specifically at retirees with no professional activity who hold a pension from the French base scheme, subject to conditions on contribution length or country of residence.
