Healthcare Reimbursement in Malawi: Who Reimburses What Depending on Your Situation
Key points
In Malawi, the Carte Vitale does not work: no social security agreement binds France and this country, and it is your status that determines who pays. A seconded employee in principle keeps the French scheme, reimbursed on French rates and on paid invoices, while a locally employed worker depends on their employer's arrangements. The self-employed, retirees, students and spouses choose between the CFE (Caisse des Français de l'étranger, the French social security fund for expatriates) and international insurance. There is no French representation on the spot, and France Diplomatie reminds us that without insurance access to care may be refused: medical repatriation must appear in the contract. The summary table sets out each case.
- No France-Malawi social security agreement: the country is absent from the CLEISS list (the French liaison centre for international social security) of States bound to France.
- No French embassy or consulate in Malawi: the Consulate General in Johannesburg is competent for procedures.
- CFE youth offer (under 30) and main offer: price on quotation, depending on country, age and family situation.
- CFE membership before departure or within 3 months: immediate cover; afterwards, a 3-month waiting period before age 44, 6 months from age 45.
- Secondment: in principle 24 months; reimbursement on French social security rates, within the limit of the expenses incurred.
Service being launched: an advisor reviews your request and gets back to you to clarify your needs.
The Principle: A Country Outside Any Agreement
CLEISS publishes the list of States and territories bound to France by a social security agreement. Malawi is not on it: there is no coordination between the French scheme and the local arrangement, no aggregation of periods and no cross-coverage. Care received on the spot is billed to the patient, who advances the costs.
Four actors can step in depending on status: the employer’s local arrangement, the French scheme maintained for a seconded employee, the CFE and international health insurance. The decision is made before departure; the notions of reimbursement basis, coverage ceiling and medical evacuation help with comparison.
France Diplomatie (the French foreign ministry’s travel advice service) describes a contract covering medical costs, surgery, hospitalisation and medical repatriation as essential: without it, access to care may be refused, even in a life-threatening emergency, and the French embassy cannot cover any costs. How facilities operate is described in the healthcare system in Malawi.
Locally Employed and Seconded Employees
The locally employed worker depends on the cover offered by their Malawian employer: the contract specifies the nature of the arrangement, the care network, ceilings, exclusions and evacuation. The pages read for this guide describe no national health insurance scheme accessible to expatriates; any local arrangement should be confirmed with the employer. Supplementary international cover or the CFE covers what the local arrangement does not provide, notably medical repatriation. The glossary defines the local contract.
The seconded employee of a French employer remains attached to French social security. According to Assurance Maladie (the French national health insurance), in a country without an agreement they are affiliated to both the French scheme and that of the host country; secondment is in principle limited to 24 months; care abroad is reimbursed on the basis of French rates, within the limit of the expenses actually incurred, on paid invoices and proof of payment (form S3125 or a request on the ameli account). Many employers add an international group contract. See seconded or expatriate and the glossary entry on seconded employees.
Self-Employed, Retirees, Students and Spouses
The self-employed worker or digital nomad has no employer to insure them: they choose the CFE, international insurance or both, depending on age, medical history and budget; see digital nomad insurance. The retiree can join the CFE offer for retirees (no health questionnaire and a contribution that depends on age and the plan chosen according to the home page) or take out international insurance. Assurance Maladie reserves form S1 for moves within Europe; for temporary stays in France, registration with the Cnarefe requires 15 years of contributions to a French compulsory scheme (10 years for those affiliated before 1 July 2019).
The student under 30 can join the CFE youth offer (price on quotation) or take student insurance; they check the requirements of their institution and their residence document. See student insurance abroad. The spouse and children are dependants at the CFE or insured on a family contract; see expatriate family insurance.
The CFE: Membership, Waiting Periods and Reimbursements
The CFE offers a youth offer (under 30), a main offer (over 30), an offer for retirees and a France-stay offer (price on quotation, for those who already hold local cover and wish to be covered only during stays in France of fewer than three consecutive months). Contributions vary by country of residence, age and family situation; the CFE simulator gives the exact rate.
Joining before departure or within three months of settling in opens immediate rights; beyond that, the waiting period is three months before age 44 and six months from age 45. No health questionnaire is requested, a member cannot be excluded, and maternity requires conception after membership.
Abroad, reimbursement applies as a rate or flat amount on the actual expense, specific to each country. The CFE’s example: for a GP consultation in France, social security reimburses 70% of the tariff whereas the main offer reimburses a higher share. See CFE reimbursements and CFE contributions.
First-Euro Insurance, Tourist and Return to France
International first-euro insurance reimburses actual costs within the limits of the contract (ceilings, deductibles, waiting periods, exclusions) and can offer direct payment, which avoids advancing the sums. In Malawi, where France Diplomatie publishes neither a list of hospitals nor an emergency number, the insurer’s assistance organises referral and evacuation. It generally requires a medical questionnaire; see medical questionnaire and history and expat health insurance.
The tourist takes out, before departure, travel insurance with assistance, checking the length of cover, medical evacuation and the exclusions. A visa has been required since 3 January 2026 and the stay lasts three months in total; the insurance must cover the planned duration, including in the event of an extension. The French embassy advances no costs. For a long trip, see long-stay travel insurance.
On returning to France, access to social security requires stable and regular residence, with a possible waiting period that can be lifted depending on the situation; the CFE retiree offer has none. See return-to-France insurance and reducing the waiting period.
Item by Item: Consultation, Hospitalisation, Medicines, Dental, Optical, Maternity
- Consultation: paid on the spot then reimbursed according to the contract (French rates for the seconded employee, country scale for the CFE, actual costs for a first-euro contract).
- Hospitalisation: to be planned with the insurer; direct payment or assistance avoids the advance of costs, and complex cases fall under an evacuation.
- Medicines: to be bought from a reliable pharmacy with a prescription in international non-proprietary names, without buying on the street; reimbursement according to the contract.
- Dental and optical: according to the scales and ceilings of the CFE and international contracts; to be confirmed with the employer for a local arrangement.
- Maternity: at the CFE, conception must come after membership; international contracts generally apply a waiting period; local follow-up depends on the employer’s arrangement.
- Evacuation: provided only by the assistance provider or the insurance contract; to be checked before departure, as no French representation is present on the spot.
The reimbursement guide gives general benchmarks.

Frequently asked questions
Does the Carte Vitale work in Malawi?
No: Malawi is not on the CLEISS list of States bound to France by a social security agreement. A seconded employee can be reimbursed on French rates, on paid invoices; other profiles go through the CFE or private insurance.
What does social security reimburse for a seconded employee?
According to Assurance Maladie, care abroad is reimbursed on the basis of French rates, within the limit of the expenses actually incurred, on paid invoices and proof of payment.
Who should you contact in case of a problem in Malawi?
There is neither a French embassy nor a consulate in Malawi: the Consulate General of France in Johannesburg is competent. The insurer's assistance is the first point of contact in a medical emergency.
Can a French retiree use form S1?
Assurance Maladie reserves the S1 for moves within Europe. For Malawi, the retiree chooses the CFE or international insurance, and can register with the Cnarefe for stays in France subject to contribution conditions.
What is the CFE waiting period?
None if membership takes place before departure or within three months of settling in; beyond that, three months before age 44 and six months from age 45. Maternity requires conception after membership.
