Expat insurance and health insurance in Indonesia

In short
Health insurance is not required for a tourist visa or a short stay in Indonesia, but it is required at two levels for a long-term resident. Any foreigner residing more than six months on a KITAS must in principle join the public BPJS Kesehatan scheme. In addition, several long-stay visas (retirement KITAS E33F, Second Home Visa E33, remote-worker visa E33G) require private health insurance in the application file, with a minimum amount specified for some of them. Failure to join BPJS effectively blocks KITAS renewal in practice; a visa application without compliant insurance risks refusal.

Local healthcare system
A two-tier system: ≈2,900 public hospitals (RSUD) and more than 1,700 private facilities, concentrated in Jakarta, Surabaya and Bali. The national scheme Jaminan Kesehatan Nasional (JKN), run by BPJS Kesehatan since 2014, covers more than 260 million people, mostly Indonesians. Only KITAS or KITAP holders can join, with access limited to the chosen network — with no medical repatriation nor care received outside Indonesia. The vast majority of foreign residents turn to the private sector. Indonesian hospitals provide no care without a prior guarantee-of-payment agreement.
Legal and visa obligations
- BPJS Kesehatan mandatory beyond 6 months (KITAS/KITAP), legal basis Perpres 82/2018 (amended by Perpres 64/2020 and 59/2024). Employee (PPU): employer contribution 4% (max 480,000 IDR/month) + employee 1% (max 120,000 IDR/month). Individual participant (mandiri): 35,000 to 150,000 IDR/month depending on the class.
- Retirement KITAS (E33F, “lansia”): from age 55-60, income ≥3,000 USD/month, balance ≥2,000 USD, health and life insurance ≥25,000,000 IDR (≈1,600 USD). See expat retirement insurance.
- Second Home Visa (E33): deposit ≥2 billion IDR (≈126,000 USD) or property ≥5 billion IDR; an insurance certificate valid for the entire length of stay is required in the file — no minimum amount is published by the Directorate General of Immigration (unlike the retirement KITAS E33F, which sets ≥25,000,000 IDR).
- Remote-worker visa (E33G), since 1 April 2024: income ≥60,000 USD/year, balance ≥2,000 USD; international health insurance mandatory (simple travel insurance is not accepted), required to cover hospitalisation and emergency care for the full length of the visa — no minimum amount published. Social security agreements with France
Indonesia is not among the countries with a social security agreement in force with France (unlike South Korea, India, Israel, Japan or the Philippines).
- No scheme coordination, no cross-border cover of health costs.
- Voluntary affiliation to the CFE or private insurance needed for cover recognised by a French system.
- BPJS Kesehatan, even when mandatory, does not replace CFE or international cover: it only covers the network chosen in Indonesia.
